[0:19] (JBover) pero, como mínimo, ya es el PRIMERO,

[0:20] (JBover) no sólo la introducción de nuevas ideas que creemos originales

[0:20] (JBover) pero también somos perfectamente conscientes

[0:20] (JBover) que sólo pueden hacerse visibles en su lanzamiento final.

[0:28] (JBover) and even some considered this initiative

[0:37] (Enrique) - New diseases: 30 new diseases have been identified in the last 20 years,

[0:37] (Enrique) including AIDS/HIV (1983) and hepatitis c (1989).

[0:37] (Enrique) - Discovering of new technologies for diagnosis and therapeutics of nephrologic diseases.

[0:37] (Enrique) - New drug therapies for hypertension, transplants, or dialysis, such as cyclosporine, erithropoetin or calcitriol

[0:38] (Enrique) - Also, consumers have more access to health information and knowledge,

[0:38] (Enrique) and this knowledge is changing the patient- provider relationship,

[0:38] (Enrique) because patients want more participation in the decision making process related with their care.

[0:38] (Enrique) The use of resources is also influenced by the ethical imperative to treat patients, the increasing social demands, the need to ration the treatments, and the degree of competition between patients for scarce resources.

[0:38] (Enrique) The ethical imperative to treat patients explained the establishment in 1965 of the Medicare program in the U.S. to provide health insurance for the elderly.

[0:38] (Enrique) Medicare was extended in 1972 to the disabled under 65 years of age.

[0:38] (Enrique) The disability provision became the basis for Medicare coverage of end-stage renal disease (ESRD) patients.

[0:39] (Enrique) The population of 10 000 ESRD beneficiaries when the entitlement began in 1973

[0:39] (Enrique) was quite different from the more than 300 000 beneficiaries of the 2000.

[0:39] (Enrique) This increase will be the result not of an inadequacy in the health care system,

[0:39] (Enrique) but, in words of Dr. Hutchinson, a Professor from the McGill University Health Center of Montreal

[0:39] (Enrique) "one of the most startling success stories in 20th-century ".

[0:39] (Enrique) The treatment of end-stage renal disease is a dramatic technical success:

[0:39] (Enrique) many people are alive now who otherwise would have died.

[0:39] (Enrique) But it can also be understood as a partial failure of the health care system

[0:39] (Enrique) because as many as 20 percent of cases of acute renal failure are attributable to drugs.

[0:40] (Enrique) Since dialysis and kidney transplantation are expensive and benefit relatively few individuals,

[0:44] (Valde) tal como los entendemos y los realizamos,

[0:44] (Valde) desaparecerán y que la trasmisión de conocimientos se realice por esta fantástica vía de comunicación que es internet.

[0:44] (Valde) Sin embargo esa realidad no debe estar muy lejana.

[0:44] (Valde) Hoy día nadie hace búsquedas bibliográficas

[0:44] (Valde) si no es por esta vía,

[0:44] (Valde) y el almacenar revistas científicas

[0:45] (Valde) esta siendo sustituido por su consulta en internet.

[0:45] (Valde) Hay que felicitar a los organizadores de este congreso en internet
