Discussion
Neurilemmoma is a relatively frequent type of benign tumor -1.2 to 10% of soft tissue tumors- and only lipomas and angiomas are more common.4 The are usually less than 5 cm. in diameter and only 13 larger cases have been reported. 4
If always arises from a peripheral nerve which is usually easy to dissect.1,5,12 In our case the nerve of origin was not identified.
Pathologically, it is an encapsulated tumor with two different cell types: Antoni type A 3,4,7,8,9 with spindle cells in a palisade formation and an interstitial substance which forms the Verocay bodies, and Antoni type B,3,9 which has irregular cells and a loose myxoid component.2,9 It is most common for both cell types to be found in the tumor6 which originates in the peripheral nerve Schwann cells.3,4,7,8,9,13
The case is original because of the considerable amount of bone involved. There are only 31 cases in the literature consulted, none of which involve the radius.11 A review of this literature suggest that the tumor may become malignant althought this does not occur in our casse.10
